{"id":3259,"date":"2012-09-05T06:00:21","date_gmt":"2012-09-05T10:00:21","guid":{"rendered":"http:\/\/www.patheos.com\/blogs\/rachelmariestone\/?p=3259"},"modified":"2012-09-05T06:00:21","modified_gmt":"2012-09-05T10:00:21","slug":"blessed-are-the-uncheerful","status":"publish","type":"post","link":"https:\/\/www.patheos.com\/blogs\/rachelmariestone\/2012\/09\/05\/blessed-are-the-uncheerful\/","title":{"rendered":"Blessed Are the Un-Cheerful?"},"content":{"rendered":"<!DOCTYPE html PUBLIC \"-\/\/W3C\/\/DTD HTML 4.0 Transitional\/\/EN\" \"http:\/\/www.w3.org\/TR\/REC-html40\/loose.dtd\">\n<html><head><meta http-equiv=\"content-type\" content=\"text\/html; charset=utf-8\"><meta http-equiv=\"content-type\" content=\"text\/html; charset=utf-8\"><\/head><body><p>Every so often I hear the insinuation that women (like me) who advocate for \u2018normal\u2019 childbirth are inordinately self-focused (even <a href=\"http:\/\/www.dailymail.co.uk\/femail\/article-1304278\/Planning-home-birth-Sorry-youre-just-selfish-reckless-A-parenting-expert-mother-threes-deeply-provocative-view.html\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">selfish<\/a>) and that women who are dissatisfied with the treatment they\u2019ve received in hospitals during labor are <a href=\"http:\/\/www.feminagirls.com\/2012\/08\/09\/welcoming-the-arrival-of\/\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">\u201cuncheerful\u201d<\/a> and, possibly\u2013according to the women in controversial pastor Douglas Wilson\u2019s life\u2013<a href=\"http:\/\/www.feminagirls.com\/2012\/07\/06\/natural-and-cursed\/\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">confused theologically<\/a>.<\/p>\n<p>Don\u2019t get me wrong: Ricki Lake\u2019s <a href=\"http:\/\/www.amazon.com\/Never-Say-Finding-Life-That\/dp\/1451627173\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">memoir<\/a>, at least as it concerns childbirth, definitely looks at the birth experience as if it is all about her. But while there\u2019s no question that medical advances (and, yes, c-sections!) save lives, it\u2019s also hard to contest the fact that medical interventions occur at rates that are simply unjustified.<\/p>\n<p>September 3, Labor Day, launched <a href=\"http:\/\/www.columbiamissourian.com\/stories\/2012\/08\/31\/improving-birth-hopes-raise-awareness-rally-sept-3-reduce-unnecessary-induction-unnecessary-c-sections\/\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">\u201cEmpowered Birth Awareness Week,\u201d <\/a>which, sponsored by ImprovingBirth.org, aims to raise people\u2019s consciousness concerning the notion of <a href=\"http:\/\/www.improvingbirth.org\/the-evidence-shows\/\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">\u201cevidence-based maternity care,\u201d<\/a> the less than radical notion that what happens during birth (ie, continuous fetal monitoring, mandatory IVs, NPO rules that prohibit eating and drinking) \u00a0should be medically indicated, not routine, and supported by sound medical research.<\/p>\n<p><a href=\"https:\/\/wp-media.patheos.com\/blogs\/sites\/484\/2012\/09\/thinkingwoman.jpg\" class=\" decorated-link\" target=\"_blank\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-3260\" title=\"ThinkingWoman\" src=\"https:\/\/wp-media.patheos.com\/blogs\/sites\/484\/2012\/09\/thinkingwoman.jpg\" alt=\"\" width=\"500\" height=\"773\"><\/a><\/p>\n<p>Henci Goer\u2019s 1994 book, <a href=\"http:\/\/www.amazon.com\/Obstetric-Myths-Versus-Research-Realities\/dp\/0897894278\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">Obstetric Myths vs. Research Realities: A Guide to the Medical Literature,<\/a> and her popular-level <a href=\"http:\/\/www.amazon.com\/Thinking-Womans-Guide-Better-Birth\/dp\/0399525173\/ref=pd_bxgy_b_text_y\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">Thinking Woman\u2019s Guide to a Better Birth<\/a> make the research clear: inductions, mandatory continuous monitoring, and \u201conce a c-section, always a c-section\u201d are not justified by the evidence. But in the popular imagination, in malpractice hearings, and, unfortunately, in insurance reimbursement rates, practitioners are better off doing something rather than nothing.<\/p>\n<p>Even though I\u2019m a doula who believes birth is beautiful and even enjoyed laboring my children the old-fashioned way, I\u2019m not going to say that it is easy or painless, because it is neither. Giving birth demands as much physical, emotional, and spiritual strength as a woman has, plus whatever she strength she can pull from the people around her, which means, of course, that the people around her have to be willing to offer that strength. The doctor who was present for the last 5 minutes of my first (12 hour) labor didn\u2019t have that strength. Neither do many labor and delivery nurses, who, because of their training and because of hospital structures, simply <a href=\"http:\/\/www.improvingbirth.org\/the-evidence-shows\/\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">have never seen<\/a> a truly natural or \u2018physiologic\u2019 birth. My friend Annie who works as a nurse in a prominent university hospital says that L&amp;D nurses are often loathe to handle the intense need for time and attention that an unmedicated birth requires. C-sections and epidurals make things much easier for the professionals.<\/p>\n<p>Adapting birth culture to meet the needs of people other than women and babies has been happening for a long time. In the semi-historical novel <a href=\"http:\/\/www.amazon.com\/The-Midwife-Gay-Courter\/dp\/0595282806\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">The Midwife,<\/a> New York City\u2019s famous <a href=\"http:\/\/en.wikipedia.org\/wiki\/Bellevue_Hospital_Center\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">Bellevue Hospital <\/a>(which housed the nation\u2019s first-ever maternity ward) is described as a boon for obstetric education and research: no longer would doctors and medical students have to wait around in filthy tenements to learn from the labor and deliveries of poor women; they could collect the poor women there, get them cleaned up (which usually involved a pubic shave and disinfecting routine, later discovered to increase infection rates) and observe their births in a scientific manner: <a href=\"http:\/\/www.amazon.com\/LYING-IN-WERTZ-Studies-life-women\/dp\/0805206159\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">great for medical education, but not so great for women,<\/a> who suffered greater rates of puerperal fever at the hands of the \u2018medical men\u2019 than at the supposedly dirty and ignorant immigrant midwives.<\/p>\n<p><a href=\"https:\/\/wp-media.patheos.com\/blogs\/sites\/484\/2012\/09\/101926434.jpg\" class=\" decorated-link\" target=\"_blank\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-3261\" title=\"101926434\" src=\"https:\/\/wp-media.patheos.com\/blogs\/sites\/484\/2012\/09\/101926434.jpg\" alt=\"\" width=\"300\" height=\"429\"><\/a><\/p>\n<p>In their 1973 feminist classic, <a href=\"http:\/\/www.amazon.com\/Witches-Midwives-Nurses-Contemporary-Classics\/dp\/1558616616\/ref=sr_1_1?s=books&amp;ie=UTF8&amp;qid=1346608741&amp;sr=1-1&amp;keywords=witches+midwives+and+nurses+a+history+of+women+healers\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">Witches, Midwives, and Nurses: A History of Women Healers,<\/a> Barbara Ehrenreich and Deirdre English point out that while women practitioners took an extremely bad rap (the <a href=\"http:\/\/en.wikipedia.org\/wiki\/Malleus_Maleficarum#Major_themes\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">Malleus Maleficarum,<\/a> which was a textbook for witch trials, focused on women healers and midwives as in league with the devil), they were, in fact, practicing evidence-based medicine\u2013using time-tested herbal remedies, for example\u2013at a time when many university trained physicians (virtually all male, of course) believed that illness was caused by an imbalance of \u201chumours\u201d and were inclined to perform dangerous, pointless, and often deadly procedures based on untested theories.<\/p>\n<p>Which brings us back to why some people (like me) are a little bit \u201cuncheerful\u201d about where medicalized birth has gone: the discussion would be different if all these c-sections (33%, or more than double what the WHO suggests as an absolute maximum) actually resulted in safer outcomes for mothers and babies. But they don\u2019t: the rate of maternal death has doubled since 1990, for example, with the US ranking #47 worldwide in maternal health outcomes. Women who are black <a href=\"http:\/\/newamericamedia.org\/2011\/06\/maternal-mortality-rates-increase-for-african-american-women.php\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">have four times the risk of dying <\/a>as a result of childbirth than women who are white. That\u2019s a justice issue.<\/p>\n<p>As I said before, there is no question that certain medical interventions save lives. <a href=\"http:\/\/www.who.int\/bulletin\/volumes\/85\/10\/06-039289\/en\/\" class=\" decorated-link\" target=\"_blank\" rel=\"nofollow\">5-15% of women will need c-sections<\/a> (in sick irony, many of those women are live in places where they\u2019re unlikely to get them.)
